What's more fun than recovering from a full spinal fusion? Trying to ambulate into a hotel bed bc there isn't a recliner available, & you end up putting too much pressure on your left arm & your scapula (shoulder blade) gives out on you. Your arm then becomes locked as shown in the pic & you're having your parents take pics bc you have to figure out how to pop your scapula back into place but can't see. It was a very different not being able to see. This wasn't the first time something like this has happened; it is the first time since surgery. After seeing the pics I basically had to visualize the anatomy in my head & think about what direction to move my shoulder. After a few tries I got that bad boy back in place. It was stuck in or over my rib. It was the first time I ever yelped & felt grinding on my ribs. I was able to move my arm again after. One might think all would be good. Not the case. Next was the muscle spasms which were pulling on my neck.

Today I saw my pain doc & the appt went well (as always). We primarily discussed the surgery & he watched the video of my funky walking. He then checked my reflexes at the knee & they shot out like nothing I've had before. There was so much lumbar pain, & I about flew out of my seat frim it. I started crying & said, :What the heck was thay?" Idk what the heck it is but we are all are in agreement with starting with PT for gait training.

This is one of my new favourite exercises for scapular rotation. If a patient has discomfort or any of the above complaints with their shoulder and you suspect that it may be due to poor scapular mechanics, then see if they can perform this. -

I especially like it because you have some resistance to work against which could function as either 1) feedback for the patient, sometimes it’s easier to produce the movement and appropriate mechanics against a little bit of resistance (shout out my neuro physios) and 2) this is a great way to progress scapular strengthening because it allows you to add resistance throughout that whole scapular rotation instead of relying on over head pressing movements which may not be as well tolerated. -

Also nothing against upper fibre biased shrugs at an angle but most people don’t have issues with their arm 30 degrees away from their body, this may be a harder but more useful alternative. Give it a try and I’d love to hear feedback 🙏

The turtle’s shoulder blade is inside the rib cage, and the ribs are part of the shell as seen by embryologists when they compared the muscles and bone development of the Chinese soft-shelled turtle, chicken and mouse. 
This means that a turtle’s shell is more than an outer covering. It is an integral part of the animal’s body.

There are 3 ways your shoulder blade can move:
1. out/in also called protraction/retraction and teachers promote the latter a lot, the usual cues are “wrap the shoulder blades, or broaden the shoulder blades”, although retracting is important as well in some poses😲 a typical example is the transition from high plank to chaturanga - going from a protracted state to a retraction💁‍♀️ protraction makes your shoulders safe while you are bearing some weight on your arms and is mostly done by those nice saw shaped muscles on the side of your rib cage called serratus💪 Also try these movements in cat/cow🤗
2. up/down or elevation/depression - happens when you shrug shoulders up and the press them down🤷‍♀️ the elevation is necessary for a straight handstand or pincha and the depression feels great for chest opening in backbends
3. last one is upward and downward rotation - while raising the arms up, the bottoms of shoulder blades rotate away from each other
All these movements are never separate and they happen together in each yoga pose😱 Learn to master them - especially the first two - the last one happens automatically😅

Keep pressing the balls of your feet into the palm like you’re accelerating 🏎 To make it more intense, you can straighten your knee and push the balls of your feet into the palm.

🇬🇧 Before performing any exercise, it is important to have an optimal posture to solicit the trapezius lower fibers. The shoulders should be lowered and slightly oriented backward and attention should be paid to the activation of the lower fibers of the trapezius. The movement then consists of forming a Y with the arms extended above the head.
The fact of varying the tools of realization (band, weight, TRX) solicits the fibers in different ways and avoids to adapt too much to an exercise.

The scapula (or commonly called shoulderblade), play an important role on shoulders mobility. So, being able to control them in their full range of motion must be a priority for everyone. • • •

During CARs (Controlled Articular Rotations), we should move the scapulas in all 4 directions: in protraction (forward/extend shoulderblades), in retraction (backward/squeeze them), in elevation (up to the ears) and in depression (distant from the ears). Bring the tension in the body and isolate them to make effective CARs.

Right handed tennis player complaining of pain at the top of his shoulder only when he served, never when his racquet was below his shoulder. I checked his external rotation first and he had adequate strength/rom.
In the first video, at the top you can see his shoulder blade doesnt help the humerus as much as his left shoulder. When he comes down his shoulder and shoulder blade aggressively come down. 
In the second video I had him slide a foam roller up the wall with his arms. This successfully got his serratus anterior to get the shoulder blade sliding up the ribcage as his arms elevated. It also let him control his decent a bit better. Still has work to do, but said the pain went away.
The next step for him would be to translate this shoulder blade action to his arm pick up in his serve.

Our shoulder segments link into a system of segments and the ability of the shoulder joint to maintain its designed space is contingent upon the demands you place on it and how other segments effect its ability to function.

If we don’t move our shoulders regularly they can get rusty...so to speak, achy, down right cranky even and for some, painful to move. One way to help maintain the space and fluid motion of the shoulder capsule is to move it slowly through it’s full maximal end range of motion with rotation (Spina’s controlled articular rotation).
- - 
Once you lose the capacity to rotate the capsule you lose a lot of 3d real estate for shoulder function. You want the ability of the shoulder capsule to move without other segments of the body having to move to get the motion.

My client Bob is working through an advanced shoulder CAR warm-up after his yoga class. He has a 3lb weight on his wrist, a band over his shoulder and a yoga block between his knees. They all add tension (load) into the system to help stabilize, support, strengthen and squeeze out as much motion as possible and as safely as possible.

The band not only adds tension, it blocks the shoulder blade...and keeps it from creeping up towards the ear. It helps to disassociate the shoulder capsule segment from the shoulder blade segment. If those two segments (bones) move in the same direction at the same speed the net motion is zero. The band reinforces independent motion of the segments so the joint can FEEL motion (relative joint motion). Move better, Feel better, Live better!💪🏻

🇫🇷 Avant la réalisation de quelconque exercice, il convient d’adopter une posture optimale afin de solliciter les fibres inférieures des trapèzes. Les épaules doivent être abaissées et légèrement orientées vers l’arrière et l’attention doit être portée sur la traction des fibres inférieures du trapèze. Le mouvement consiste ensuite à former un Y avec les bras en extension au-dessus de la tête.
Le fait de varier les outils de réalisation (élastique, poids, TRX) permet de solliciter les fibres de manière différente et évite de trop s’accommoder a un exercice.

Neste caso estou levando a Transferência de força mais para baixo (fazendo uma ação anti-natural da máquina - pois ela tem uma tendência a jogar o Vetor vermelho mais para cima pela ação mecânica), quando fazemos a Remada Baixa conseguimos trabalhar os Dorsais, Rombóides, Trapézio fibras médias/Inferiores, Redondos Maiores e Menores, Deltóides posteriores e etc! .

Está ação anti-natural com o Vetor de força verde é mais direcionada por uma simples prática de colocar o Polegar na máquina (reparem no vídeo) e fazer uma pressão para baixo e ao mesmo tempo colocar o Grau de Mobilidade em Extensão do Torácica junto ao movimento de puxar o “remo”.. .

Está variação ajuda a manter os músculos mais Tensionados no trabalho total, e assim deixando menos espaço vago sobre estas contrações musculares! .

Using the theraband for feedback helps to change the movement pattern rather than simply working a muscle. Clients report less pain between shoulders, more comfortable whilst feeding and less tightness through the front of their shoulders.

Tight upper shoulder muscles, may actually be due to a lengthened, weak muscle, where stretching doesn't help - always get assessed first to make sure each exercise is the right one for your body.
