Magnetic resonance cholangiopancreatography (MRCP) on a patient with pancreatic cancer.
MRCP, which introduced in 1991, has an established role in the investigation of many biliary disorders, serving as a non-invasive alternative to endoscopic retrograde cholangiopancreatography (ERCP). It makes use of heavily T2-weighted pulse sequences, thus exploiting the inherent differences in the T2-weighted contrast between stationary fluid-filled structures in the abdomen (which have a long T2 relaxation time) and adjacent soft tissue (which has a much shorter T2 relaxation time). Static or slow moving fluids within the biliary tree and pancreatic duct appear of high signal intensity on MRCP, whilst surrounding tissue is of reduced signal intensity.
🍕DIET & ULCERATIVE COLITIS:
🍩Generally, the most important thing is to eat a nutritious and balanced diet to maintain your weight and strength, and to drink enough fluids to prevent you from becoming dehydrated. Even so, you may find that certain foods affect your symptoms.
🍦For example, when you have a flare-up, bland, soft foods may cause less discomfort than raw vegetables or spicy foods. You may also find that eating too much fibre can increase the urge to go to the toilet. The urge to open the bowels is usually caused by inflammation in the lower colon, but, as fibre adds bulk to faeces, it can act as a trigger and make the urgency worse. So, it may be helpful during flare-ups to reduce the amount of fibre you eat and perhaps go on a low-fibre diet.
🌭In order to make sure your diet remains healthy and well balanced, it is important to get advice from your doctor or dietitian before you make any major changes. Once the flare-up is over, you should try to increase your intake of fibre again. Fibre is useful because it keeps the colon healthier as well as providing fuel for beneficial bacteria.
🍟Some people with UC have noticed that dairy products trigger their symptoms – although research suggests that, in general, people with UC are no more likely to be lactose intolerant than the general population.
🍅You may sometimes need nutritional supplements to replace low levels of vitamins and minerals. This can happen when you have a poor appetite, do not eat enough, or when you have ongoing diarrhoea. Many people with IBD are also deficient in vitamin D. Vitamin D is formed by the action of sunlight on the skin and is also found in foods such as eggs and oily fish. It is important for the immune system and is also needed for healthy bones.

I had a meeting with a counselor at Henry Ford College today and I left very worried, stressed and disappointed. Today was a miserable day.
After I take my prerequisite classes to start the radiology program, I will then be put on a 3 year waiting list.
Right away I became irritable and felt lost inside my head. It was completely out of my control and I hated it. I’ve wanted to become an X-ray technician since before I started Massage Therapy.
I spent a good majority of my day depressed and anxious thinking about it. The more I thought about it though, the more I realized that I have no strong desire to pursue any other career.
I am going to work my ass off and stick to this. I will graduate this program. I will succeed. My life and way of living at this very moment is temporary. Knowing that is what keeps me going.
I can sit here and dwell on the negative for no reason, or I can push through and wait and do what I can until then. I’m an impatient person. I want things now. Today. It’s unrealistic.
I have to work hard and stay focused. Success doesn’t happen overnight. 🔹To differentiate biliary from portal venous gas in the liver, look at the location: biliary gas is central while portal venous gas is peripheral.
🔸The case I showed you of biliary gas also had small bowel dilatation and a stone at the transition point - this triad is known as Rigler’s triad.
🔹Dr Leo Rigler was an American radiologist.
🔸Rigler’s sign is also named after him - this is a sign of free peritoneal gas on abdominal x-rays when the bowel wall is seen very clearly as there is gas on both the luminal and peritoneal side.
🔹The image I showed you also had two features seen in ulcerative colitis - thumbprinting which is a sign of colonic wall oedema, and dilatation due to toxic megacolon. The colon had perforated and in addition to the Rigler sign of pneumoperitoneum, there was also the falciform ligament sign. ⠀⠀⠀⠀⠀⠀⠀⠀⠀
🔸Another triad associated with the biliary system is Charcot triad - this is a triad of signs in ascending cholangitis and consists of intermittent fevers or rigors, jaundice and right upper quadrant pain.
🔹Returning to gas in the liver, I also showed you a case of gas in the portal venous system due to arterial bowel ischaemia with a paper-thin bowel wall - I am going to be doing more teaching on bowel ischaemia in the next few weeks.
