CASE STUDY - When is it time to consider getting a scan?

My client experienced a popping sound and strain during a movement a month ago and hasn’t improved since🤕
She has had a treatment before and self-recovery - but tonight was her first treatment with me🤞🏼
After testing, we found that the ischial tuberosity (sit bones under your glutes) was painful when being pressed🍑

What did I do?💆🏽‍♀️
I put her pelvis back into neutral to lessen the stretch and offloaded muscles attached to the ischial tuberosity to reduce some of the pulling.
There was slight improvement but because of these symptoms and duration of condition I suggested an ultrasound

So what’s my point I hear you ask??🙄
When little to no improvement has occurred after trying a few things for a while (without a diagnosis), and if something doesn’t feel right, we must re-evaluate and getting a scan can give you your answer on what’s going on👨🏼‍⚕️
This way we can choose the RIGHT treatment method for you to then have a quicker recovery🤸🏽‍♀️

Lateral raise variation || 👉 Lateral raises can be great for our shoulders when performed correctly but can be detrimental to our shoulders when performed incorrectly. ➖➖➖➖➖➖➖➖➖➖➖➖➖➖
Adopting a slow and controlled approach to the lateral raise helps focus the work where intended (mostly anterior shoulders) and creates a strong foundation for our mind-muscle connection. 
Adding a pause shortly after initiating each rep ensures the shoulder is firing prior to moving through its range. 
This exercise also challenges our rotator cuffs that assist in stabilising our shoulder joint. ➖➖➖➖➖➖➖➖➖➖➖➖➖➖
KEY CUES:
1. Torso & Glutes ON! Don’t swing/arch.
2. Focus on lifting with your shoulders. 
3. Lift your elbows in line with your shoulder (assuming your range of motion allows pain free movement).
4. Slow and steady
5. Weight doesn’t need to be heavy, I’m using 3kg DB/ @coach_lester 10kg. You can feel this exercise with no weight!
6. Quality over quantity, instead of counting reps - complete quality reps for 40 seconds, 4 sets. 1-2 minutes rest between sets.

The 2018 Australian Youth Olympic Team was made up of 88 athletes aged between 15-18 year olds, along with 66 staff. The games themselves hosted the best young athletes in the world across 32 sports. Our Aussie team came home with 4 Gold🥇, 8 Silver🥈 and 4 Bronze🥉! _

Stretching after a run! Who does it? I sometimes rush off after exercise and commit to do it later.... sometimes we just can’t fit everything in! I couldn’t commit to a training run today after work and having to pick Harry up from Eltham Play house. Instead I did a smaller run to pick him up ! I was getting strange looks running with an empty pram 😜 ( This women has forgot her baby or she is a complete lunatic) but hey I got a run in! Don’t make it hard on yourself people’s, do what you can and that is enough! Here are some basic stretches to commit to after exercise or when ever you can fit them in.  Hold for 1-2 min each • Hamstring Stretch - keep leg straight, supporting foot pointing to outstretched leg.
• Quad Stretch- make sure your pelvis is pushed forward, knees are close together
• Glute Stretch- Hold onto a fence and cross one foot onto your thigh, make sure knee is turned out, lean back and hold. • Calf Stretch- poi t both toes to front, bend front knee and keep the back leg straight
•Hip Flexor with side bend- lunge forward onto a step, push pelvis forward reach over with same arm to leg to stretch out ribs too.

As a highly accomplished senior scientist and immunologist, Shala finished her PhD in immunology and melanoma cancer research in 2002 at Monash University. For her postdoctoral fellowship Shala was offered a position at Shriners Hospital for Children (Portland, Oregon, USA). As an author, Shala has published scientific articles in a number of nationally and internationally recognised journals.

After 10 years of postdoctoral fellowship in cancer research and immunology, Shala sought to start her second Bachelor Degree in Health Science (Naturopathy) at Endeavour College of Natural Health in 2012.

Shala is the Lead Researcher at MIOG and balances clinical practice with consistent development of our extensive database of evidence based research. She is dedicated to the comprehensive analysis of supportive interactions and research for cancer patients.

The posterior chain is the group of muscles at the back of our bodies from the achilles tendon all the way up to the sub occipitals at the base of the skull.

If you are sedentary and sitting down most of the day I can guarantee your posterior chain is weak which can lead to leg cramps, strains, sprains, sciatica, lower back pain, dislocations, disc herniations, thoracic outlet syndrome, carpal tunnel, nerve impingements, headaches etc etc. 
As a Myotherapist and Strength and Conditioning Coach I see it with my new patients numerous times a day! 90% of the conditions I treat stem back to a weak posterior chain. 
It is crucial to have strength and conditioning in the posterior chain as they hold us upright, keep our joints in proper alignment, provide stability and allow us to perform every day tasks and movement patterns without causing injuries to our body. A proper functioning posterior chain also decreases the likelihood of developing myofascial trigger points!

The Deadlift often gets a bad wrap, however if performed correctly it can be the most beneficial exercise as it targets the posterior chain muscles all at once.

Like any excerise I prescribe try to keep the weight to a minimum and keep constant tension on the targeted muscles. Aim for 8 repetitions with the last two being a struggle and perform 4 sets.

The knee is what we call a ‘consequent joint’. Often people feel pain in the knee due to what’s happening in joints above, at the hip or low back, or below, in the ankle and foot. If corrections aren’t made in these places, your pain is likely to persist or return after some treatment on the knee. Females are slightly more likely to experience knee pain, as are people who work in the trade industry or athletes.

Pain has a way of ingraining itself with movements and we can retain what is, essentially, a ‘pain memory’. The movement that initiated traumatic pain or movements that have continuously been associated with our pain can become ‘hardwired’ into our pain experience. .
For me, spinal extension has always been one movement that is linked with my pain. Therefore it has often been something that I have avoided doing. .
However, avoidance should rarely be a part of a long term treatment plan. Getting comfortable with loading my spine into extension wasn’t a quick process. Nor was it one in which I didn’t have to address many unhelpful beliefs and fears about my back. .
But I’m enjoying the process and the fruits of my continued efforts!

The last two days I didn’t get up and do my morning routine. On Friday night we had friends over and I ended up staying up late having a chat with a friend. I didn’t get to bed until the wee hours of the morning so I intentionally turned off my alarm as slept a little longer before work the next day. On Sunday my partner had taken the day of so we had a sleep in together. Normally I would get up and do it anyway but I think the late night the night be for had taken it’s toll and I just could not get out of bed to get it done. 
Having those two days without it though really made me realise how much I enjoy it, how much of a difference it makes to my day, and how much getting up early helps me start the day on the right foot. 
Going forward, after my October challenge, I will keep up my morning rituals, however there may be days like this past weekend where I sleep in instead, and given the circumstances I think that’s just fine.

A pretty astute observation by Willy Wonker.
Some people say some brutally stupid things and the non-squat, squats are bad for your knees, people are up there with flat-earthers. 
I actually had a patient recently who had 3 knee surgeries and had been unable to squat without pain for 5 years. Because of his pain the Physio  told him not to squat, the surgeons told him he could not squat, so did his medical doctor. I sat him down and put him into a perfect box squat position and gave him 2 very simple cues then I asked him to stand. It was the first painless squat he had done in 5 years. He has been painless and squatting ever since. What were the cues? How did I assess his Biomechanics to know what to do?
