The movement system contains a lot of things, a lot more than just the motor output; the movement result we can see. It is our cognitive processes, our perception of the environment (and ourselves), our sense of purpose, self-efficacy etc. and not least our emotions. When we train with our horses we often think about it as training different movement exercises, we are concentrating on getting the correct motor output. Just as an experiment, what would happen if you thought of your training with your horse as affecting your horse’s emotions? Every movement pattern is connected to certain emotions. Can you come up with some examples? What emotions do you experience when you do certain movements? As we’re both social animals, our emotions often reflect one another. There is a reason we say the horse is our mirror. And we are the horse’s mirror! When you’re doing difficult, effortful stuff, do you feel stressed, tense and not enough? Or do you feel powerful, badass and playful? Also questions to ask your horse. Embora já tenha feito esse post, considero ele tão importante que sempre será atual.

Tenho visto sistematicamente nas academias um erro bastante comum, o qual eu também cometia quando comecei a praticar #lpo. Qual é? Eliminar a transição da primeira para segunda puxada e transformá-las e uma única puxada.
.
Durante a transição da primeira para segunda puxada, a barra deve PASSAR PELA COXA e, principalmente, PELO QUADRIL. Ou seja, ela DEVE SER PUXADA PARA DENTRO, a postura ligeiramente modificada (tronco verticalizado - posição da cadeira) para então gerar a maior potência e acelerar a barra para cima em uma combinação sincronizado e extensão total de quadril e remada alta (high pull) - vídeo lado direito.

O grande problema está em não fazer essa transição e já iniciar o movimento, desde a posição inicial, com a maior velocidade, não puxar a barra para dentro (passando pela coxa e quadril) e batendo com o quadril na barra e projetando ela à frente - algo parecido com o vídeo da esquerda.

*CUIDADO! Quem acelera a barra é o quadril, DEPOIS os braços entram com a high pull, de forma sutil. Esse time (sincronismo) é fundamental para o sucesso do levamento. Esse tipo de exercício é muito complexo, demanda MUITO TEMPO E REPETIÇÃO para aprender e, pode ser lesivo sim, se vc não praticar da maneira adequada. Se você está evoluindo muito rápido, desconfie da técnica. Peça auxílio de um profissional qualificado.

If someone has shoulder issues, pain, restriction, stiffness, loss of strenght (like me now and then 😊) due to whatever reason we have to be careful what kind of exercise we give to her/him. You can see quick changes in my shoulder mobility into medial rotation depending what I do. Can I affect some kind of structural imbalances just by doing a movement twice? There is no chance for that 😊. Is it nervous system what can react very fast and alter my movement? Yep 👍🏻. If our brain get various danger messages what does our brain do? It can restrict mobility, decrease strength and speed of movement, co-contract muscles, alter muscle timing etc. Would it be good for me to do right over-head press? May be not or at least I have to modify exercise by pushing left arm down for example. We can not forget lumbopelvic area, hip etc either. Which side? We have to test that. If we do not have test how can we treat or rehab? And our assessment system evolves by time. Our body and mind are so fantastic and plastic. A bit of a long read with the first half or so probably more suited for a physical therapist audience, and the last couple of paragraphs geared towards the general population. If you're interested feel free to skip through to whichever parts may be relevant to you. I find that fleshing out my thoughts through these course reviews serve as effective reminders of what I had learned, but also allow prospective/current patients to see what kind of thought processes I have in my treatment approach.

I liked this course because I felt like it brought me back to the roots of my identity as a PT, which is to be a critical thinker. With so many different types of courses and treatment approaches out there, to what end does a PT need to learn the seemingly myriad approaches/styles of providing rehab? This technique, that tool, this manual therapy, that reinvention of the wheel. After a while it seems that a lot of continuing education camps are taking the same scientific principles and research and are basically coming up with different languages to express the same central messages. Personally speaking, moving forward I'd like to take courses that foster critical thinking skills and as @michael.ray.dc puts it, are education heavy as opposed to being technique heavy. Take courses that teach you how to think. #movementsystem #movementexam #movementanalysis #physiotherapy

Continuing my "back rehab". Deadlifts has been in my practice more than 20 years. Few years ago weights were double but so was my immobility! 😇 I wish not to go back having that body and the ability to (not) move but I still enjoy the benefits of DL. Finally in the home stretch of Residency and finishing up my last few projects. Big thanks to @babbyjd for making me my own Y-Balance kit, and for “volunteering” to be the subject for my Wellness Screen Live Patient Exam. For this LPE, I created a sports specific screen for hockey players to look at various movement impairments and lower extremity deficits that might predispose an athlete to injury. Based on the results from the screen, James was given a corrective exercise program to address his ROM, strength and motor control deficits.

Movement is real medicine! Few days ago I hurt my back a little and could barely move my spine. Today it still hurt when I moved but after rolling around for a while it started to feel better and better. Every now and then there will be pain and your ability to move is constricted.  It sucks but most of the time there is always something you can and should do. (Shared with permission) This is rehab session number 7 post Achilles repair, and our first trial of running. That’s right, I said seven visits. I’m finding that in these days of high deductibles and coinsurances, my self-pay physiotherapy practice model is more cost-effective than going through therapy with insurance-based providers. I actually charge LESS than traditional clinics and I get paid MORE, because there is no middle man. That affords me the time to treat each patient individually, one-on-one, and focus on their own specific needs. I don’t have to see people three days/week (that is a tactic to make more money) or treat another patient at the same time (another tactic) to make business ends meet. I can spread visits out just the right amount for each person, and I can continue to provide care and expertise long past any arbitrary cut-off date set by insurance. So I can be there after things get back to normal, and I can leave a body better than I found it. Does your PT communicate regularly with your doctor? [Patients]

If your physician had referred you to a Physical Therapist (PT), it’s important to have them communicate often about your progress.

Most PT's will be sending your Doctor or Surgeon regular progress notes about how you are doing in physical therapy.

The communication should go both ways. If you see your surgeon for a check up, and they give you specific instructions, make sure they directly communicate that with your PT as well.

We want to help you get out of pain, move better and recover as quickly as possible from your injuries!

In the morning I was reading about growth and fixed mindset by Carol Dweck or today I was reading actually Angela Duckworth who was talking about her stuff. She wrote very well that we all have both sides and many of us who are with growth mindset can still have very fixed thoughts at first. It is the next thing that happens when you get up, which today was literally true. And you know things got serious when shirt got off! 👊🏻 I am sure I will get this one and it will happen soon.  With or without the shirt! And maybe just little bit of more mobility or the ability to compress myself while press thru the shoulders.

Moving through a stretch wakes up proprioceptors in our joints, muscles, tendons, and ligaments. These propioceptors allow us to recognize where our body is in space. Stretching a muscle or joint in one plane/direction DIRECTLY helps improve mobility in the other 2 planes/directions.

If you’re trying to protect an injury, then the good old fashion static stretch is great too. 
_
Our body craves movement, and to ensure that we can maintain a movement pattern, we need to have healthy range of motion (ROM). There are those that avoid stretching at all cost and don’t believe it’s as important as strengthening.

Keep your range of motion to age gracefully. Consistent stretching is key and learn how to control your ROM.

When ever I stumble with that feeling I go back to the very basics. Maybe not all the way to the beginning but to those skills and drills which are pretty easy. Refining those few days or weeks and then go back what ever you were doing. For me this works every time and I believe it is not only me who gain from this approach. When ever I stumble with that feeling I go back to the very basics. Maybe not all the way to the beginning but to those skills and drills which are pretty easy. Refining those few days or weeks and then go back what ever you were doing. For me this works every time and I believe it is not only me who gain from this approach. Weight vest made it interesting 🤔 brigde is not one of my strengths but now that I have done more or less daily, it is slowly getting more comfortable even when feeling stiff otherwise.

When asking our patients and clients to perform a squat, we have seen countless variations. Most of those variations are contributing to excessive stress placed either on the knees, and/or the lumbar spine. 
_
If we had a penny for every time we’ve seen excessive medial knee collapse or a poor hip hinge, we’d be rich. 
_
Here we have a proper squat with the feet in a neutral position. She demonstrates an appropriate hip hinge allowing the gluteals to be loaded, reducing the work and stress placed on the quads and patella. Her trunk is also at an appropriate angle, further reducing the load on the lumbar spine. 
_
All the muscles of the hip and legs are doing their specific job to promote a safe and efficient movement.

Are you collecting data just to collect data? Do you know how to correctly interpret all the data you collected in order to create an appropriate treatment plan?

When you are a new grad, you should err on the side of collect more vs. less data. This will help  to build your library of knowledge for beginning to categorize patients into separate differential diagnoses and clinical patterns.

As a new grad, the picture doesn't always become clear as you are taking all of your measurements. So take the time after the patient has left to organize all of the data you have collected.  Each patient you see is like a puzzle; make sure you have put all the pieces together in order to give them the best treatment possible.

Frontal Plane Lunge

Using your arms as a driver to the opposite (contralateral) direction provides this lunge with a great adductor stretch and thoracic mobility. 
_
Gravity can be used in this dynamic stretch as a load  and help prime the functional movements of the hip and surrounding muscles. 
_
The reason why this is a great movement? The adductor muscles are often overlooked when priming movements, ultimately contributing to nagging groin strains and other hip injuries. 
_
Hopefully this will make you see the traditional lunge in a new light.
