Your body does not work in isolation, especially when we talk about the neuroendocrine system. [gut health]: a little over a year ago when @jessmacphoto helped Lauren and I get some amazing photos, I was not myself. Experiencing anxiety for the first time and not realizing it, unable to put on real pants during the work day due to bloating (thank goodness for working from home), and feeling paralyzed when it came to what to eat, I needed help. About a week after this photo, and amazing sorority sister of mine (who’s now a functional practitioner) asked if she could help me with my suspicions of SIBO and Candida. First off, we need to give credit to the body because sometimes we don't need to treat the root cause in order for an issue to completely resolve. Sometimes we just need to get the body close enough for it to kick in and do what it knows how to do, which is heal yourself. Other times, we just need to get out of the way and let the body heal itself!

However, the closer you get to addressing the root cause, the higher the chances you will have of that complete healing.

That could mean treating the hormone deficiency in a post-menopausal woman who is having hot flashes, instead of just giving them a drug to manage the hot flashes.

That could mean addressing the emotional issues surrounding a man in his seventies who is not achieving erections he wants to anymore, instead of just giving him Viagra.

That could mean removing foods that are inflammatory triggers for patients with autoimmune conditions such as rheumatoid arthritis, instead of just giving them oral anti-inflammatories.

Trying to treat the cause shifts healthcare from a protocol based management system to  truly individualized healthcare. You can't treat the cause unless you're actually get to know the human being behind the label of that health condition. Probably the best place to start.

A woman in my online health membership, The Healthy Mama Hive, recently told me her doctor put her on a hydrocortisone cream because he believed she had low cortisol however, it made her feel worse. 😞 So I dug up her old @dutchtest and sure enough she doesn't have low cortisol: she has low FREE cortisol - big difference. What this doctor never measured was her METABOLIZED cortisol, which was trending high! This poor mama didn't need more cortisol, she needed to work on other things to support her adrenals and the way her body is clearing the cortisol she was making, (she cleared it too quickly). • 
Free cortisol is what is UNBOUND and what's biologically available and active in the body. Only when cortisol is “free” or un-bound is it able to enter cells and affect tissues, but it only represents about 1 - 3% of total production of cortisol. Therefore, it's not a marker for the total production of cortisol in the body - for that we need to look at METABOLIZED cortisol. • 
Metabolized cortisol is indicative of your total cortisol production for the entire day - sort of like your "bucket" of cortisol. It tells you how much cortisol your adrenals have made in the day and it tells you if you are making it or not. • 
A saliva test doesn't give you metabolized cortisol, (only free) and therefore this test doesn't give you the full picture of your adrenal function. Hence, your practitioner may give you the WRONG TREATMENT and end up making you feel worse - like what happened to my client. • 
And what's even more awesome about the DUTCH test is that when we see the free and metabolized patterns don't match (when they're going in opposite directions) this can give us clues. So with my client the clue from her cortisol pattern is that she may be insulin resistant, (she is), she may have hyperthyroidism (she doesn't), she may be obese (she isn't). So we now know we have to dig deeper into insulin and blood sugar as well as support her adrenals and HPA axis. • •

Been one off those weeks...My computer is playing up making work impossible, printer broke down, London office  @minna_hepburn with no internet/landline, husband away, my little guy at home injured and just too many things to do, pulling me mentally into too many directions. I think it’s time for re-evaluation. 
Over the years I have learnt  to respect and love myself and admit I am not a super human. 
I know I am not the only one. Us women, we juggle with too many things these days. To be the perfect mother, wife and have a successful career. 
Can we do it? I can honestly put my hand up and say it’s certainly not working for me most days 🤯
Yet, I still try. And fail. 
We all respond to stress differently. And stress has multiple faces. It can be real life and death situation, good stress ( yes you can have too much of a good thing) , food related, environmental... it’s all the same. 
Stress is definitely my biggest enemy and surprisingly have felt little symptoms starting to re-surface... gut issues and panic attacks. Yes,  those two are so interlinked. Ever heard of the gut- brain axis? Vague nerve? Your bloody is bloody intelligent 🤓 
As @robynpuglia said “ you cannot supplement your way out of stress”, I need to do some serious lifestyle interventions. 
So be kind to yourself this weekend. Do things that make you happy. Breath. Nourish yourself and do some gentle exercise. Sleep. Have a bath and read a book. 😘

Thyroid replacement is not well tolerated if cortisol levels are too low.
When estrogen is high in a man, testosterone replacement will push more estrogen and make that situation worse.
Unopposed estrogen (giving estrogen without progesterone) can increase cancer risk and worsen symptoms of progesterone deficiency

There are many more examples I could give, but I feel like I've made my point. The neuroendocrine systems work in balance with each other and this is extremely important not only when testing but also when considering different treatment options. Sometimes it is possible to just treat one system and achieve the desired results, however more often than not two or more systems need to be simultaneously addressed. Not all of these need to be addressed with hormone replacement therapy, and it is possible to use herbs, supplements, nutrition, and lifestyle to support different systems while another system is being treated.

This is another big reason why I don't believe in protocols, especially in the hormone world. No two patients are ever the same, which is why protocols for the masses don't work near as well as individualized treatment programs.

This is the beauty of the neuroendocrine system, and the fact that human physiology is a spectrum and not a cliff. 
This especially occurs when a minor shift in a hormone (usually an androgenic hormone) that causes an increase in production of a large number of other hormones.  Examples of this are DHEA, GH/IGF-1, and testosterone.

This is why herbs and supplements specific to the neuroendocrine system can have profound effects on quality of life.  These herbs and supplements are not typically causing massive shifts in hormones, which typically only occurs with actual hormone replacement, but rather small minor shifts that still have massive impact on your quality of life. The important part of this is that these supplements and herbs need to be specific for each patient and where in the endocrine system they are having issues.

What I mean by this is the following: if a man is having symptoms of low testosterone, just giving him 'testosterone boosting herbs' won't do much if his issue isn't that he isn’t producing enough, but rather that he is turning his testosterone into estrogen.  In that case, supplements/herbs directed at reducing that conversion of testosterone to estrogen would have more impact.

Same with women: if a woman is having symptoms of estrogen dominance, just lowering estrogen may not be the best approach if her issue isn't that she has too much estrogen, but rather not enough progesterone.  If that were the case, progesterone boosting supplements/herbs would have more impact. 
While there is a time and a place for bioidentical hormone replacement therapy, there is also a time and  a place for supplements/herbs/lifestyle/nutritional changes that can have massive impacts on your quality of life.

The important part of this is that these supplements and herbs need to be specific for each patient and where in the endocrine system they are having issues. What I mean by this is the following: if a man is having symptoms of low testosterone, just giving him 'testosterone boosting herbs' won't do much if his issue isn't that he isn’t producing enough, but rather that he is turning his testosterone into estrogen. In that case, supplements/herbs directed at reducing that conversion of testosterone to estrogen would have more impact. Same with women: if a woman is having symptoms of estrogen dominance, just lowering estrogen may not be the best approach if her issue isn't that she has too much estrogen, but rather not enough progesterone. If that were the case, progesterone boosting supplements/herbs would have more impact. In a recent post, I discussed how “stress” is SO MUCH MORE than a feeling of overwhelm (tho that can certainly be part of it). It can include ANYTHING that triggers our nervous system to be ON GUARD, and tips our system into overload including: .

Exposure to toxic chemicals in buildings, outdoors, and in the products we use to clean as well as personal hygiene 🤢 .

An inflammatory diet high in processed foods, sugar, and caffeine, as well as undiagnosed food sensitivities 🍭 🥤 ☕️ .

Over exercising and under eating. 🏃🏻‍♀️ 🥗 .

Hostile living and/or working environments 🏠 🔥 .

All or nothing beliefs like “busyness = worthiness” , or “good people do _”, people pleasing, and lack of boundaries. 😃 😡 .

Comparing ourselves to others and pushing/forcing things to be a certain way to the point of exhaustion. 🔨😞 .

So when we are told that our symptoms are due to “too much stress”, there is likely some truth to it, and also A LOT TO UNPACK! 😨 .

That is why I work 1-on-1 with women who are experiencing chronic low energy, insomnia, weight change, changes in digestion and food/caffeine tolerance, hypoglycemia, and increasing anxiety and overwhelm. Because stress is big and broad. It’s like been going on for some time. And changing habit patterns and pace of life isn’t always the first place we look to, but is often 🔑! .

However, I also want to create resources with the EP series that can help you take action in rewriting stress to REST! And deep breathing is A PROFOUND healer! Mine stressramte klienter spørger tit ind til min holdning omkring stress og motion, da mange (desværre) er af den holdning, at motion er fy fy, hvis man er stresset.

Jeg kan desværre ikke give noget entydigt svar, da det er meget individuelt og afhænger af din grad af stress.

Her rammer vi nemlig ind i en af de allerstørste misforståelser om stress. Mange tror fejlagtigt, at man først er ramt af stress, når man er kommet så langt ud, at energien er kronisk forsvundet og kroppen er ramt af kroniske smerter, så man kun kan ligge hjemme under dynen i fosterstilling.

Den tilstand er ikke stress, men adrenal fatigue - binyretræthed - udbrændthed. Her lander du først, hvis du har overhørt/ignoreret din krops signaler i meget lang tid! Her er motion slet ikke en mulighed, da kroppen er sat mere eller mindre ud af spillet 🤒🤒🤒 og kun har ressourcer til, at du kan trække vejret, hjertet kan slå, spise og sove. Her virker kun ro og atter ro.

Inden du lander der, har din krop været i stressberedskab i forskellige trin.

Først bliver du udsat for en stressorer. Stressorer er ting i dit liv, der aktiverer din stressrespons. Fx. fraværet af direkte omsorgspersoner i din barndom, høje lyde,  skilsmisse, dødsfald, ændring af arbejdsopgaver, forelskelse, den mandlige udløsning under 6, dårlig ledelse etc.

Når stressrespons bliver udløst, gøres din krop på et splitsekund klar til flugt eller kamp.

Undslipper du uden at føle skam efterfølgende, falder kroppen til ro igen, når du har rystet oplevelsen af dig eller motioneret stressresponsen ud af kroppen.

Trækker den mentale kamp ud, er motion et godt middel til at brænde stresshormonerne af. Fordi stressresponsen har brug for en fysisk udladning, på samme måde som hvis du skulle løbe eller kæmpe for livet, for ikke at brænde sig fast i krop og sind.

Her skal du dog være opmærksom på, at det skal være kortvarig og eksplosivt motion med udgangspunkt i dine forhåndenværende ressourcer, så du ikke overbelaster dig og bliver skadet.

Motion forløser nemlig stressresponsen og træner både din fysiske og psykiske robusthed. Din krop er nemlig designet til at blive brugt og udfordret fysisk for at holde hjernen frisk😊

Woop woop! One of my favorite neuro-endocrine-immunological compounds, HISTAMINE! Brace yoselves'! Histamine is an amino acid derived (histidine) neurotransmitter that serves MANY functional roles in the body. It's an excitatory, inflammatory immune modulator that increases blood vessel dilation for immune cells to migrate to target tissues. 
Histamine can directly alter serotonin, and often those with high histamine have symptoms of low serotonin (depression, carb cravings etc. see my serotonin graphic). Histamine can also modulate other neurotransmitters (dopamine, glutamate etc.) Histamine is metabolized intacellularly by histamine n-methyltransferase (HNMT), which means proper methylation is key (see my article on MTHFR). Extracellularly histamine is metabolized by diamine oxidase (DAO). Here's where it gets fun, high estrogen (remember the ebv estrogen connection) can decrease DAO thus exacerbating histamine symptoms. 
Also candida/yeast and histamine share a metabolic pathway as both are broken down into a compound known as acetylaldehyde. Acetylaldehyde is very toxic to the nervous system and must be metabolized into acetic acid by aldehyde dehydrogenase (ALDH), aldehyde oxidase (AO), and xanthine oxidase (XO). Because of this fungal overgrowth in the gi tract can also exacerbate histamine symptoms. #plotTwist it gets better because acetylaldehyde inhibits methionine synthase which is a rate limiting enzymatic step in the methylation cycle. So ironically enough histamine metabolism problems, due to excess acetylaldehyde can further exacerbate histamine issues in a self perpetuating fashion. 
We could really do a whole show on this, but ultimately your body is always synthesizing and breaking down histamine. The question becomes how much histamine building blocks are you consuming? Do you have genetic polymorphisms causing metabolism issues? What environmental factors are triggering histamine? And other current physiological influences are present.

It was lovely to touch base at the BANT Local meeting yesterday with a special guest speaker from @regeneruslabs . As a Nutritional Therapist we can often over complicate things when it comes to #functionaltesting for our clients. Numbers itself don’t tell the full story! We shouldn’t look at data in isolation. We need to be asking ourselves:
1. Why did I choose this test?
2. What am I trying to find out? 
3. Pick up 3-4 points from the test results that are relevant to this case.
4. How is this going to help my client? 
Not all clients are going to be happy spending the extra money on testing, however there are some cases where it is absolutely necessary to get to the bottom of the health issues. Yesterday we were discussing the #dutchtest and what to look out for in this hormone test without over complicating things. It was a really useful exercise as it helped me interpret numbers and understand what it means. This is going to take a lot of practice and further education watching the useful videos on www.dutchtest.com . The steroid pathway diagram shown here looks very complicated 🙈 but learning more about the sex hormones, metabolites, cortisol and cortisone yesterday, made me feel more comfortable and keen to learn more about the different pathways and how they relate to particular symptoms and test results. We touched on organic acid testing which is also part of the Dutch Complete test and has very useful markers such as vitamin B12, vitamin B6, Glutathione, dopamine, norepinephrine/epinephrine, serotonin, melatonin and oxidative stress. What some people believe to be a hormone production issue, I believe to be either a communication error, or a metabolism error.

As you age, the production of sex hormones from your ovaries or testes decreases, despite an increasing message from the brain (LH & FSH) to make more hormones. In men this is called andropause, and women this is called menopause. However, this concept cannot be extrapolated over to the adrenal glands, which is commonly done. Only in rare instances do you see a drastic rise in ACTH, the message from the brain telling the adrenal glands to make more cortisol.

Don't get me wrong, I treat the HPA axis, which includes the adrenal glands, in many of my patients who are suffering from excessive fatigue. The difference, is that I am not trying to get the adrenal glands to come back online and produce more cortisol, rather I am trying to get them to hear the message from the brain better so that they produce more cortisol on their own. Put differently, I'm not treating an adrenal gland that is fatigued, I am treating one that is not listening very well.

But, the big caveat is that if testing reveals we don't actually have a production issue, but rather a clearance issue, then that requires a different approach.  Typically, if sufficient amount of cortisol is being made but free cortisol levels are low, then looking for the cause of this is going to move the needle the most.  The biggest culprits of this are hypothyroidism, 
In some regards, the treatment for the communication issue is very similar regardless of which way you look at it, however there is huge power in understanding why you are doing a treatment and what effect you are trying to create. This taps into the emotional and psychological aspect of healing which is highly underrated in today's medical system.

And in order to get into the brain, it needs to pass through the blood brain barrier. But serotonin and dopamine precursors use the same carrier to get into the brain, do they are always competing for a spot.

Many of us (ahem 🙋🏼‍♀️) go for reward more often since it provides a lot of short term please. These are things like 🍩sugar, ☕️caffeine, 🍷alcohol, drugs, 🍕processed foods, online shopping, 🤳🏻scrolling social media, buying the 1,234th health program 😄. But in the process of getting that short term hit, we are ruling out long term contentment. Contentment seems boring when you could have excitement right?!

✴️Stress & Self-care ✴️ As someone who is constantly on the go, pushing my body to the limit & never switching off is very familiar to me 🚵🏋️🏊🏃I don't think I am alone in this...many of us are humans doing not human beings..😌 What I am not so adept at is sitting still & listening to my body 🙇💆but in the last year, I have had to learn to slow down, my body signalling that it was struggling to keep up with demands, I began experiencing some very unpleasant symptoms (that's for another post 🙄) So after a mentally tough day at work, rather than go to the gym as planned, I spent the evening journaling, meditating,  spending time on the beach & with my hubby ✍️🕉️⛱️ 👫🙏 SELFCARE is not a luxury, it's a health necessity...our bodies are like batteries ➕➖we can't keep depleting our bodies & not refill them with rest, recovery & selfcare..it's a matter of physiology...to build our rest & digest (parasympathetic) system & our stress response (sympathetic) system to ensure we don't overstay our welcome in one state, which can be problematic ☯️ #workinprogress #skininthegame

This goes back to the notion that health is a spectrum, and not a cliff. The one end of the spectrum for HPA axis dysfunction is from normal to Addison's disease, which is where the adrenal gland makes little to no cortisol, and the patient typically requires cortisol replacement starting as a child.

But, there is a lot of grey space between normal and Addison's disease, and where a patient resides in that grey space suggests which treatments they will more likely respond to.

As a general statement, everyone will benefit from basic lifestyle and nutritional changes that support a healthy HPA axis: // get 5-10 minutes of sunlight directly on your face within the first 30 minutes of waking up, and when the sun is highest (10-2pm)
// block blue light in the evening (f.lux, night shift mode, blue light filter, blue light blocking glasses, etc.)
// sleep in a completely dark room with as little electronics as possible.
// relax in the evening for at least 30-60 minutes before bed (try reading a book!)
// cut out caffeine by 1pm each day

Beyond this, therapies and interventions can range from basic vitamin and mineral support, to targeted herbs, to cortisol replacement therapy. And even each of these layers of treatment, there are variations in which herbs, or which vitamins/minerals, or even which form of cortisol can be used as treatment, based on the individual circumstances.

There is no blanket treatment protocol, because you are a dynamic human being that has different physiology compared to the next person with the exact same state of health.

If your doctor doesn't individualize your treatment, please find a new doctor.  I understand that may be hard and daunting, and the next one you find might not be good either. The point is to continue to advocate for your health.

Triggering your stress response under specific conditions can absolutely serve you well.  Delivering a tight deadline at work, scooping up your toddler as they run into danger, or that moment before you step up on a platform to address a room full of people. Focused, alert, energised! ⠀

Activating our fight-flight response, as was intended. ⠀

Yet when left untamed, when activated repeatedly, our stress response can cause untold damage - to our bodies, our health and our ability to thrive. ⠀

We'd be well to think of our stress response like fire. It can serve us well with the right conditions but triggered long term and we're dealing with an outright emergency. ✨

To paint a picture, it was said by our older female instructor with crazy long gray hair and in a thick french accent — and.it.STUCK. But did I stop drinking my one cup even knowing my adrenals were probably dead? of course not!

Knowing what we have to do to heal doesn’t hold much weight until we take action. I justify coffee again and again because I love the aroma, taste, effect, and routine of it. It’s a really tough habit for me to change, but I’m taking steps - woot woot!

If you’ve seen my monday instastories you know I’m setting little goals for myself each week. My major goal is to rebalance my hormones. My #estrogen and #progesterone are at the levels of a postmenopausal woman 🤦‍♀️ so that’s not cool. Gotta pretty much increase everything, my body has been running in a deficit for a few years so with one change at a time I’m hoping to regain balance! I suffer significantly from #raynauds and my toes have already been giving me trouble and it’s not even cold yet, so I need to get my shit in order before the temps actually do drop! .

“Caffeine is a whipping lash for your adrenals” . To paint a picture, it was said by our older female instructor with crazy long gray hair and in a thick french accent — and.it.STUCK. But did I stop drinking my one cup even knowing my adrenals were probably dead? of course not! . Knowing what we have to do to heal doesn’t hold much weight until we take action. Lyme disease is caused by the spirochete bacteria, borrelia (strains vary by region). The borrelia bacteria has the crazy capability of altering its genome in order to change its exterior shell antigens in order to avoid being detected by the host's immune system. These modified surface proteins can trigger autoimmunity in the host. 16 proteins on the borrelia bacteria have been shown to cross react with thyroid tissue, thereby triggering thyroid autoimmunity such as Grave's or Hashimoto's thyroiditis. Lyme can also trigger chronic inflammatory response syndrome, or CIRS, see my post on mold illness. The bacteria also produce protein "cocoons" called biofilms that protect the bacteria from antibiotic treatment and immune system defense mechanisms. What some people believe to be a hormone production issue, I believe to be either a communication error, or a metabolism error.

As you age, the production of sex hormones from your ovaries or testes decreases, despite an increasing message from the brain (LH & FSH) to make more hormones. In men this is called andropause, and women this is called menopause. However, this concept cannot be extrapolated over to the adrenal glands, which is commonly done. Only in rare instances do you see a drastic rise in ACTH, the message from the brain telling the adrenal glands to make more cortisol.

Don't get me wrong, I treat the HPA axis, which includes the adrenal glands, in many of my patients who are suffering from excessive fatigue. The difference, is that I am not trying to get the adrenal glands to come back online and produce more cortisol, rather I am trying to get them to hear the message from the brain better so that they produce more cortisol on their own. Put differently, I'm not treating an adrenal gland that is fatigued, I am treating one that is not listening very well.

But, the big caveat is that if testing reveals we don't actually have a production issue, but rather a clearance issue, then that requires a different approach.  Typically, if sufficient amount of cortisol is being made but free cortisol levels are low, then looking for the cause of this is going to move the needle the most.  The biggest culprits of this are hyperthyroidism, 
In some regards, the treatment for the communication issue is very similar regardless of which way you look at it, however there is huge power in understanding why you are doing a treatment and what effect you are trying to create. This taps into the emotional and psychological aspect of healing which is highly underrated in today's medical system.

Treat the communication or clearance issue, not fatigue.

Most social circles understand that excess body fat is detrimental to health, especially when the weight is found around the abdomen.  Most of the time, this occurs in males, as differences in hormones typically result in women storing excess fat in their hips and thighs.

Unfortunately, many symptoms that patients deal with from a hormonal stand point, are more drastically affected by body fat % than other conditions. Clinical growth hormone deficiency is still rare, but that doesn't mean subclinical GH deficiency is.  Physiology is a spectrum, not a cliff.

Subclinical GH deficiency can cause:
// lack of self-esteem
// lack of concentration
// poor stress tolerance
// cold intolerance
// anxiety
// muscle weakness
// many many more symptoms

The short take home from this is that if you are suffering from symptoms of low growth hormone, and have some pounds to lose, you need to put in the hard work to lose weight in order to treat the cause. 
The difficult thing though, is that sometimes growth hormone deficiency causes obesity, which further perpetuates the decrease in growth hormone. And as GH helps you to build muscle mass and shed fat, low GH levels make it even more difficult to lose the weight that is required to bring GH levels back to normal.

That's why we sometimes need to palliate symptoms while you work on the underlying causes. Then as things start to shift we can slowly wean you off the hormones and let your body take control.

We are each one person who can do one thing well in each moment. And when we focus on doing one thing, it actually calms our body, balances hormones and prevents the negative effects of stress.

It’s called mindfulness. You can do it anywhere, anytime. In fact, you can even do it while you sleep! Just sleep, that’s it. Allow your body and trust your body to do what it needs to do to keep you healthy while you sleep.

Let’s continue the conversation about low thyroid function with how you can be an advocate for your thyroid health.
_
Ask your doctor to check your thyroid function every year when you have blood work done. They should test the TSH level as well as free T4 and free T3.
_
The TSH should be no more than 2.5 if you don’t take thyroid hormone medications, and if you do take thyroid medication, it should be between 0.5 and 1.5.
_
If you have not been tested for thyroid auto-antibodies then that should be done as well.
_
There are also ways to pay out of pocket for these tests so you can get the information.
_
Keep in mind that many of the possible causes of an underactive thyroid are directly related to stress in some form or another. Being human, with all the common exposures to stress we experience every day, makes it quite likely that your thyroid might not be functioning optimally.
_
Thyroid function is a spectrum – your thyroid might be under-functioning just a little bit, or it could be functioning well below what it should be.
_
And yet, when you have a standard blood test to check thyroid function, it is mainly used in standard medical approaches only to identify severe under-functioning. And the standard approach to severe hypothyroidism is to give a synthetic form of T4, the inactive form of the thyroid hormone rather than the active form your body can use.
_
Yet research shows that:
* People who have been taking synthetic T4 long term are likely to still be experiencing low thyroid symptoms.
* Women who have even slightly low thyroid function are more likely to experience a miscarriage.
_
So don’t just stop there. If you are experiencing symptoms of low thyroid function (see prior post and blog article at the link @drdoniwilson) then I encourage you to:
* Meet with a naturopathic doctor

I encourage you to be an advocate for your health. _ Let’s continue the conversation about low thyroid function with how you can be an advocate for your thyroid health. _ Ask your doctor to check your thyroid function every year when you have blood work done. They should test the TSH level as well as free T4 and free T3. _ The TSH should be no more than 2.5 if you don’t take thyroid hormone medications, and if you do take thyroid medication, it should be between 0.5 and 1.5. _ If you have not been tested for thyroid auto-antibodies then that should be done as well. _ There are also ways to pay out of pocket for these tests so you can get the information. _ Keep in mind that many of the possible causes of an underactive thyroid are directly related to stress in some form or another. Being human, with all the common exposures to stress we experience every day, makes it quite likely that your thyroid might not be functioning optimally. _ Thyroid function is a spectrum – your thyroid might be under-functioning just a little bit, or it could be functioning well below what it should be. _ And yet, when you have a standard blood test to check thyroid function, it is mainly used in standard medical approaches only to identify severe under-functioning. And the standard approach to severe hypothyroidism is to give a synthetic form of T4, the inactive form of the thyroid hormone rather than the active form your body can use. _ Yet research shows that: * People who have been taking synthetic T4 long term are likely to still be experiencing low thyroid symptoms. * Women who have even slightly low thyroid function are more likely to experience a miscarriage. _ So don’t just stop there. If you are experiencing symptoms of low thyroid function (see prior post and blog article at the link @drdoniwilson) then I encourage you to: * Meet with a naturopathic doctor * Check your cortisol and adrenaline levels so you can optimize adrenal function * Do a good sensitivity panel so you know which foods to avoid * Check nutrient levels, anti-oxidants, mitochondrial function _ It’s worth it for you to feel your best. There are certain areas where this applies, but other areas where this necessarily doesn't. One of the biggest places that this statement applies is when looking at your hormones.

Reference ranges are designed to show that the majority of the population looks like, and it only collected from a sample of 1,000 people.  If they surveyed and actually found the patients who felt amazing and were asymptomatic, the reference ranges would look more like the optimal ranges that I typically use in my practice.

Minor shifts in hormones can produce large shifts in symptoms, and this is one of the reasons that I believe this notion of optimal lab values plays out so heavily in endocrinology. The beauty of this is that specialized testing isn't required to resolve symptoms, just basic hormone testing.  Mind you, a physician trained in detecting and correlating subclinical deficiencies is imperative to this concept.

To play devil's advocate, your hormone lab values don't need to be in the optimal range in order to feel good. Again, hormone labs don't always tell the full picture, and sometimes they don't match what is clinically occurring. The key is to integrate the two for the larger context.

▶️ Both stress and leaky gut lead to nutrient deficiencies, such as not having enough iodine, tyrosine, iron, selenium, zinc, and B vitamins. These nutrients are important for the production and activation of thyroid hormone.

▶️ Mitochondria are the energy producing “engines” in each cell in your body. If they are stressed and depleted, not only are they not able to make energy, but they also can’t activate thyroid hormone.

Happy tears.  I just retested my cortisol and hormones and they're almost completely normal!! (Two years ago I was nearing adrenal failure and my cortisol measured around a 3-4 all day.) I've been battling adrenal issues and HPA axis dysregulation issues for a decade. What do I credit? Let’s start with decreased memory. Do you feel like you keep forgetting something in the other room? Forgetting names, words, what you were about to do?

It might not be dementia, yet, but it is a sign that your brain is stressed.
_
It’s possible to reverse dementia, which means it’s possible to improve your memory. And the way to do it is to recover from stress and to sleep more!
_
You see...even tho it seems like we do nothing while we sleep...we are actually doing a lot while we sleep.😊
_
Our brains clean out the plaque that causes dementia while we sleep. It’s called #glymphatics and it’s a NEW scientific discovery!
_
Plus, while we sleep, our bodies repair, our immune system protects, and our hormones rebalance.
_
We need 7.5 to 9 hours of sleep for that to happen. So if you’re not getting at least 7.5 hours, let’s start by helping you get better sleep.

How well are you sleeping? If not sleeping well, it could be your cortisol is high, or your serotonin and melatonin are low. Melatonin is made from serotonin after all.

We need some tests done to find out why you’re not sleeping. Start with the @dutchtest and may want to check NT too.

We also need to check your gut health - food sensitivities, leaky gut, gut bacteria - cuz the gut-brain axis can trigger sleep issues and dementia.

But these two people have helped me to see that I am the one who is responsible for the choices I make. It is only when we chose to take on this responsibility can we find the freedom that we crave.

What does this mean clinically?  It means we can measure how your adrenal glands respond to stress in order to gauge how effective your HPA axis is working.  This response is known as the Cortisol Awakening Response (CAR). A healthy, well-functioning HPA axis responds to stress with an increase in cortisol production. Within the first 30 minutes of waking, this means that cortisol rises sharply which helps you to wake up and be functional.

A dysfunctional HPA axis responds to stress with a blunted production in cortisol. This means that it will take you longer than 30 minutes to 'wake up', if you feel like you do at all.

The CAR is arguably the best way to assess how the adrenal glands are operating from a functional standpoint. Unfortunately, there aren't many tests that are able to check this due to testing limitation.

This is why I use @dutchtest with CAR, which provides me with a full adrenal profile including cortisol metabolites and cortisone, along with the cortisol awakening response.

For years I had to avoid any form of caffeine in order to restore my adrenal health, gut and hormones (it was a sad time 😂💔)

When I would have a sip my brain would light up 🤩 and temporarily I would feel amazing!!!....but then i’d be left feeling anxious, unable to sleep and craving MORE coffee over the next few days 🙄👎🏼

I have 3 rules though....1️⃣ It must be ORGANIC 🌿 - Coffee beans are one of the most pesticide laden crops! 2️⃣ I have to drink it BEFORE 1pm at the very latest, else I will NOT be able to sleep 😳 3️⃣ I NEVER drink it on an empty stomach (as this is a major stress for your hormones 🚫) or when i’m having an extra stressful/busy day 🙅🏻‍♀️

Once you have restored balance to your body’s systems, your next aim should be to help your body maintain that balance, no matter what stress/stressors it might face in the future.

This is what I mean by “resilience.”
_
Resilience is the ability to go out into the world and bounce back when faced with things that may have made you sick in the past.
_
It means your adrenal response is optimal, which, in turn, helps optimize your digestion, immune function, hormones, and neurotransmitters.
_
When we are resilient, we get sick less often and recover more quickly. We become less susceptible to infection, allergens, and sensitivities.
_
In other words, the ultimate goal of my proprietary system is to help you STAY well, rather than make you feel “better” when your body is, essentially, unwell.
_
This is what I refer to as bring a Stress Warrior. A warrior to stress.

And that’s what I’m busy writing about in my newest book - Stress Warrior - coming out soon!

Huge Caveat: this post by no means holds true in every aspect of medicine. Emergency and traumatic care needs to rely heavily on labs and imaging for treatment, and rightfully so!  This post is more for the chronic disease aspect of healthcare.

Within the realm that I work in, I see a lot of men and women who were refused treatment for their hormones because they were "within normal range". These men and women came to me symptomatic and suffering, but the medical system had failed them because they didn't meet certain 'criteria'. On the other side of the coin, patients come to see me after failed injections or surgeries, to later find out that previous physicians only treated off the imaging.

Some people want to blame those physicians, but I will fight tooth and nail for every single physician who .  We can't blame doctors for things they weren't taught, or the fact that certain things were so strongly taught that those molds can't be broken.  We also can't blame doctors who are doing what the insurance companies tell them they need to do in order to perform treatment.

The system is broken.  It's easier to point the finger at specific people, but it's the entire system that is broken.

One thing that doesn't get written down (and maybe it should) is that time to relax. I feel pretty good about this weekend, though. I made time to get to the gym and rewarded myself afterward with the hydromassage table. My guy and I ordered in takeout Saturday night and watched the last Sharknado movie (sooooooo terrible, and not even in a trashtasticly good way), and actually went out for a movie date Sunday (BlacKkKlansman — highly recommend!). I definitely took more time to relax & refresh this weekend than I normally do, and I’m starting this week off with a fuller cup than usual. *

Dealing with #hpaaxisdysfunction has destroyed my digestion, but I have found that these digestive enzymes have recently helped me out tremendously. While going through my “no workout” challenge to help me recover, I’ve also stopped taking all supplements. STEP 2 for Adrenal and Stress Recovery: Restore the Balance

To perform as they are meant to, ALL your bodily systems – hormones, metabolism, digestion, nervous, adrenal, and immune system – need to be in balance.
_
The reason stressors trigger ill health is that they create imbalances in these systems.
_
And because all these systems are interconnected, if one system gets out of balance, it can affect one or more of the others.
_
If this imbalance is left unaddressed for a long time, it can create a snowball effect, resulting in a vicious cycle that continually creates greater imbalance.
_
If your body is caught in this vicious cycle of imbalance, eliminating the stressors from your environment probably won’t be enough to restore health.
_
You ALSO need to restore balance to the systems that have gone out of whack. First we need to know which systems are out of balance. There are specialty panels to help us identify these, such as the @dutchtest
_
These panels are not often offered by a convention medical office, but they are essential for finding the underlying causes and what is needed to restore balance.
_
Once we know which systems need attention, restoring balance is often a two-step process.
_
First, we would restore calming support in the body. For example, we would aim to lower cortisol levels if they are too high, or increase calming neurotransmitters, such as serotonin and GABA, if they are too low. We would also take steps to calm blood sugar fluctuations and reduce any inflammation in the body.
_
Once the body has “calmed down,” we would start working on restoring natural stimulators like adrenaline and cortisol (if either is too low). These can help restore energy levels and mental focus.
_
It is essential to work on the calming support before introducing stimulating supplements, or you are likely to end up feeling worse.

Often working with my clients reminds me of some of the things I need too. Earlier this week while preparing for a client call, I made this note on her form. It really resonated with ME TOO!! And I’ve been trying to remind myself each day since of this, again and again.

The past 2 weeks have been a bit rough. Each year in August, September, or maybe as late as October, I get slammed with some intense symptoms that have usually disappeared for the whole summer. In the past, this meant struggling with those symptoms all fall and winter long in an intensified way. However, last year, being my strongest and healthiest since the sudden unraveling of my health in 2012, it didn’t continue in the same way throughout the winter.

It’s very possible we recognized the link last year to a seasonal allergy making this time of year a trigger for me. Although I don’t get any of the “traditional” allergy symptoms, the way it suddenly comes slamming in, I do believe there is a likely link. So I’m going to explore that more. Anyone else experience this?

Additionally, though, it was becoming very clear to me that I have not allowed enough rest since March. So I’ve been reprioritizing some self-care this week (even if in the small cracks in my day). Things I’m making more time for...gratitude, breathing, bedtime rituals, etc. And all of today has been about spending a slower day with my boys as summer break comes to an end this coming week. 😔

So I want to share this glimpse into my 3 step protocol, starting with STEP 1: Remove Stressors.

“Stress” is not just a feeling or mental state; it’s also a physical state. Your body becomes stressed when it cannot handle what is happening to it. Anything that puts your body in a state of stress is a “stressor.”

Psychological stress, relationship stress, injury, infection, toxins, foods that trigger you immune system and bacteria out of balance – all these kinds of stress will affect four main systems in our bodies: Digestion, immune system, nervous system, and hormones.
_
When these systems become imbalanced, that can lead to three main health issues that can trap us in an ongoing state of poor health: Adrenal distress, intestinal permeability (leaky gut), and imbalanced metabolism (blood sugar issues).
_
Every one of us is exposed to stress/stressors every day; it is impossible to eliminate all of them. However, some stressors – such as toxins or allergens in our air, food or water, or infections in our body – can trigger inflammation and dysfunction in our bodies, and it is better if we get rid of them.
_
If we don’t first remove these stressors from our environment, we’ll just end up going in circles, experiencing the same problems over and over.
_
Of course, you won’t necessarily know WHAT to remove, simply by looking at your symptoms. The only way to know for sure is to get checked for food sensitivities, toxins, metals, and infections.

Whenever I start working with a new patient, the first thing I usually do is discuss which specialty panels are available to help us identify what is most likely triggering negative health symptoms.

Then we create a plan to address any stressors we can remove and start to choose our stress exposure wisely.
