It is pretty rough to find something “grab and go” to eat when your trying to heal from HPA Axis dysfunction. Almost every food that enters your GI tract causes inflammation. It’s like the very thing your body needs for survival is the poison that is tearing it apart. 
On top of that, when your adrenals overproduce cortisol your body is in constant fight or flight, causing all of your body’s secondary functions (like digestion) to slow or stop. Here is the thing I am remembering: 1. what limited foods I CAN eat are life giving...a gift for my healing body. 2. Taking the time to make my own grain free bread, chop veggies, and prep my own meals sparks my system to begin producing digestive enzymes that aide in turning the healthy foods I can eat into nourishment.

My ‘bring me back to life’ salad.

The past couple of months have been quite challenging. Personal life has been tough, multiplied with the 101 planetary retrogrades.

What to do in these stressful times?
.
-Swap more stressful/inflammatory forms of exercise for yoga, walking, dancing and Pilates. These kinds of exercises encourage stimulation of the parasympathetic nervous system, which help keep you feeling zen and allow your body to repair and reboot.
.
-Indulge in relaxing Epsom salt baths, 1/2 x a week. Why not throw in some lavender oil to get even more relaxation in? Not only is this bath relaxing, but it also has a detoxifying effect, and may even be useful for stopping a cold dead in it’s tracks.
.
-Reduce/remove caffeine to reduce the release of stress hormones, which hinder your body’s ability to repair itself, and may even throw off your hormone balance - and if you’re trying to avoid weight gain, muscle catabolism, poor sleep, PMS and many more, you really don’t this problem! Go on a hunt for a non-caffeinated tea or hot drink blend which includes plenty of antioxidants like turmeric, cinnamon, clove etc and come up with a new sexy drink which isn’t going to quite literally, stress you!

-Meditate. Find a way to dial in to yourself. Use music or not, do it alone or in a group, at home or in nature (please be safe tho). This also encourages parasympathetic mode.
.
-Pay attention to what you’re putting into your body. This is the time of year where it can turn into a free for all. Yours truly, not excluded. Even if you’re still indulging in junk, please still take the time to nourish your body with lots of plant based foods, bone broths and slow cooked hearty stews. The last thing you want is for your body to burn out, so give it lots of powerful phytochemicals to fight the good fight for you, and to ensure your nutrient needs are met.

L’avanzare con l’età porta un fisiologico calo muscolare e neurologico, avvertibile in termini di qualità della vita e quantificabile tramite misure diagnostiche del S-Score (integrità del sistema muscolo-scheletrico) e T-Score (densità minerale ossea sistemica). ⁉️ stiamo rispettando questa “curva” dell’invecchiamento ⁉️ No. In moltissimi di noi la curva è molto più ripida del calo fisiologico, questo porterà la scienza a cambiare i parametri del “normale”. Perchè purtroppo se la popolazione peggiora il “normale” viene ridimensionato. 😱 In termini di qualità e quantità del muscolo e dell’osso, tantissimi soggetti dimostrano anche 25 anni di più.

Sedentarietà o attività fisica fatta a caso, fumo, scarse ore di sonno in generale tutto ciò che racchiudiamo (sbagliando) nella parola Stress, ci permette di auto-cannibalizzarci.

Arriverà un momento a breve dove chiunque voglia parlare seriamente di salute, che sia movimento, che sia nutrizione, avrà il bisogno di tenere in considerazione S-Score, T-Score sistemico e analisi del sistema nervoso autonomo. 
A conti fatti, senza parametrare, si va a caso.

Più del 70% dei casi analizzati in Studio HPA (fino ad oggi oltre 300), ha il proprio punteggio S-Score ALMENO 0.5 deviazioni standard sotto la norma per la sua età e sesso. In parole povere la struttura neuro-muscolare, tendinea e tutto ciò che è interdipendente dal muscolo è più vecchio di circa dieci anni.

Presto sarà la norma, come l’obesità che ora parte dal 30% di grasso ma in alcuni paesi si inizia a valutare di spostarla al 35% ...ah si, si parla di percentuali, si  può essere obesi anche a 70kg, basta avere molto poco muscolo e scarsissima densità minerale ossea.

Except when there's too little estrogen and/or progesterone (i.e. around menopause in women, and commonly low progesterone in anyways in men and women). These two sex hormones help regulate the HPA axis, so a drop in these throws things off to where we actually have a rise in cortisol! This can disrupt sleep, make us more irritable, gain weight, and slow metabolism, to name a few, actually further throwing off hormones, and of course our stress response. ⠀

What to do? Sleep before 11pm- seriously. The neuroendocrine system is an elegant, but complicated, series of interactions and feedback loops that help to create homeostasis in the body. This means that the thyroid hormones don't just communicate with other components within the same thyroid axis, but also with the other neuroendocrine axes such as the HPA axis and the HPG axis.⠀

Now, the reason for this intolerance is because active thyroid hormone (T3) increases the breakdown of cortisol into cortisone.  Cortisol and cortisone are very similar, except for the fact that cortisone is has 10-20% less activity than cortisol.  This means that if you are suffering from symptoms of low cortisol, increasing T3 levels will make those symptoms worse as you will lose some more of the cortisol to cortisone.  On top of this, under situations of stress where cortisol should increase, those with cortisol deficiencies but taking thyroid medications are more likely to have a hyperthyroid episode.⠀

As a general rule of thumb, I assess the HPA axis in 100% of patients with suspected neuroendocrine dysfunction, and end up supporting the HPA axis in one degree or another in 80% of confirmed neuroendocrine dysfunction cases. ⠀

If you've been told that you are hypothyroid but don't tolerate thyroid replacement well, find a physician who can help you heal your adrenal glands.  That may be the key to resolving your symptoms for you.⠀

Lipopolysaccharides (LPS), a feature common in Leaky Gut, can also activate this same pathway as stress and cause inflammation and adrenal imbalances! . To help our bodies deal with stress we should daily meditate, say positive affirmations to ourselves in a sacred space like...when you’re driving or when you take a shower. Coffee is the bombbbb, but let’s be adults about it...
Last week I had a client and a friend text me about coffee with VERY different experiences:
Person 1: Drinks a latte, wired 🤪
Person 2: Drinks 2 cups, takes 3 hour nap 😴

Why thooo??? 🤷🏼‍♀️ It comes down to cortisol & adrenal health... Drinking coffee = ⬆️ stress in your body. Cortisol and adrenaline get released. This puts you in a sympathetic “fight or flight” state 👊🏽 We should only be in this state 20% & parasympathetic “rest and digest” state 80% of the time, but most people are opposite. I see it with almost every client who is overly stressed out, high training, too much caffeine, under eating, not taking care of themselves. Result = HPA axis dysfunction.

When someone throws down 2-3+ servings of caffeine a day this will F things up. Drive your car without getting an oil change, replacing tires, etc. and you’ll be screwed. Everything works, until it doesn’t. Those 3 cups of coffee sure helps ya get shit done but you’ll crash and your cortisol will be high/low at inappropriate times of the day. (Why people wake up unrefreshed and reach for coffee or lay down for bed and can’t fall asleep)

There are certain situations within medicine where the body gets into a vicious cycle, and autoimmune (AI) rheumatological conditions are a prime example.  DHEA and testosterone are very commonly low in AI diseases such as rheumatoid arthritis, psoriatic arthritis, lupus, Sjogren's, etc.  Over time, cortisol also tends to be extremely low, but this is usually due to repeated oral/injected corticosteroids that conventional medicine uses to suppress flares. ⠀

A decrease in these hormones massively impact the immune system, and contribute to an inappropriate immune response.  Low DHEA, cortisol, and testosterone doesn't always cause an underactive immune system, but rather an inappropriately responding immune system.  This is the underlying cause of an autoimmune condition - overactive in some aspects, and underactive in others. ⠀

This is why I evaluate and address hormone deficiencies in my AI patients.  Just because they don't have the classic symptoms of DHEA deficiency, doesn’t mean that they won’t have extremely low levels, and that they won't benefit from DHEA or testosterone replacement. ⠀

Burn out is typically when you have the feeling of physical and emotional exhaustion. Usually due to stress from working with people under difficult or demanding conditions for a prolonged period of time. 
Burn out from a physiological approach means that resources are completely depleted and your body and mind are completely exhausted.

Sharing a post by one of the most well-versed and knowledgeable doctors on functional endocrinology @saragottfriedmd , because it is so important to understand what is causing so much stress, fatigue, brain fog, irritability, depression, mood swings, low immunity, and mental decline: many of which are a result of a dysfunctional HPA axis or Hypothalamic-Pituitary-Adrenal axis. 
A signal from the hypothalamus in the brain sends a hormonal signal to the pituitary gland, also in the brain, to the adrenal glands that produce cortisol; the stress hormone. This feedback loop is necessary for us to deal with stressful situations, but when it doesn’t rebound back to normal it becomes very problematic. Good sleep cycles, meditation, exercise, supplementing with GABA, adaptogenic herbs and magnesium, diaphragmatic nasal breathing, and other relaxation techniques like spending time in nature helps to calm down the over stimulation of the hypothalamus and reduces cortisol. The goal is to have a gradual decline in cortisol upon waking up and going to bed. This aspect of health and disease often gets overlooked in the conventional world, for a few different reasons.  The first, is that there isn't a good, effective way at the moment to measure mitochondrial activity and potential dysfunction.  The second is that there isn't a set of 'classical symptoms' associated with mitochondrial dysfunction, because the mitochondria is involved in so many different processes in the body.⠀

The one thing that tends to be consistent with mitochondrial dysfunction is fatigue.  This is because the mitochondria are the main area where energy is made in the body.  In addition to this, your sex hormones and cortisol are also made here, which can produce symptoms of fatigue when they are low.⠀

But, there's a curveball. Your thyroid hormone, which is not made in the mitochondria in the thyroid gland, is the key hormone that upregulates mitochondrial function. So, if you have a functional or cellular thyroid hormone deficiency, your mitochondria don't work as quickly as they should, contributing to excess fatigue.⠀

So, treating the mitochondria with both diet, lifestyle, and targeted supplements can be beneficial, but if thyroid is the main culprit then that needs to be addressed first. ⠀

And to throw one more curveball at you, if intracellular levels of thyroid hormone are low because chronic stress, obesity, xenoestrogens, or low grade infections are altering how your body converts inactive T4 --> active T3, then those need to be addressed in conjunction with the above.⠀

Day 2 of 3 is all about adrenal burnout. There is so much info out there on adrenal fatigue, how it’s related so closely to stress, how it weakens our immune system, affects our sleep, our food choices, is the cause of body aches, hair loss,  digestive issues, stresses out your thyroid, your hormones, causes mineral imbalances, mental fatigue, brain fog, the list is worryingly almost endless. ⚡️
But I want to talk about the other part of adrenal burnout that isn’t often included in the very medical list of symptoms. The mental, emotional and spiritual elements of adrenal burnout can be as devastating as the physical symptoms. The feeling of being so broken, you don’t know where to start. The deep, deep depression that walks alongside the exhaustion. How it’s often easier to slip further down the spiral than haul yourself out of it because you just don’t have the energy. How getting through the day is so, so hard. Just putting one foot in front of the other takes all your effort, and it feels like walking through treacle. The tears, the disconnection, the confusion, lost in a brain fog so bad you wonder if you’ll ever make it out (and yes, I’m speaking from experience).

Coupled with the time that we’re living in now, where self-worth is measured by busyness, where it’s normal to ‘push through’, to ignore or suppress the symptoms we’re feeling, that our body is creating to communicate with us, to beg us to slow down, to stop for a while. ☄️

Hashimoto’s Health Tip: Understanding Women’s Hormones

This week we've been covering hormonal issues. Yesterday, I explained positive and negative feedback loops. Today l'll take a look at progesterone.

High or Low Progesterone?

Progesterone up-regulates TPO (the enzyme thyroid peroxidase). This is why when women ovulate, their temperature goes up.

During ovulation, the surge of progesterone stimulates TPO.

When this happens there is a surge in thyroid hormones produced, that makes the temperature rise.

Women with low progesterone can also have less thyroid hormone than their body needs (even thought this isn’t always visible on a lab test.) But they might have both low thyroid and low progesterone symptoms.

Symptoms of low progesterone include heavy menstrual cycle, inability to lose weight, mid-cycle luteal phase depression, headaches, etc.

It is interesting to note that problems with progesterone are often not caused by your body being unable to make enough progesterone, rather they are caused by some sort of problem with the pituitary gland.

There are a number of axises involved with the pituitary, the adrenal-pituitary, thyroid-pituitary and ovarian-pituitary connections can all affect each other and can lead to problems with this communication system.

This is always a better place to attempt treatment before adding external progesterone which can actually make Hashimoto’s symptoms worse.

What to Do?

Obviously, this is not something that a single pill is going to correct. Rather than look for the drug or supplement to solve the problem let’s look at how hormones behave and see how we can improve that.

The physiologic effects of hormones depend largely on their concentration in blood and extracellular fluid. Disease and health problems happen when hormone concentrations are too high or too low.

First thing to do is to test to determine whether or not you have enough hormone in your body. As we saw above with progesterone therapy, this can make matters worse if you supplement when you already have enough.

If there isn’t enough, you should consult your physician to help your body get or make more.

One of the genes responsible for caffeine metabolism is CYP1A2. Your variation of this gene helps determine whether you feel like a hot jittery mess after one cup of coffee, or can chug a liter before bed without missing a minute of sleep that night.⁣ ⁣
I land in the 45% of people known as “fast” caffeine metabolizers. ⁣ ⁣
Regardless of how much we love our daily cup-o-jo (or 4 or 5…) caffeine intake should limited in people who are experiencing HPA axis dysfunction.⁣ ⁣
HPA stands for hypothalamic-pituitary-adrenal, and this is a major pathway in our bodies responsible for a myriad of functions. You may be familiar with adrenals - and the term “adrenal fatigue” is a common way of saying HPA axis dysfunction.⁣ ⁣
Caffeine is known to interact with and up-regulate this pathway, increasing the amount of ACTH and corticosterones (stress hormones) in the system. ⁣ ⁣
This is especially true with larger doses of caffeine, which is probably why we’re so jumpy after that 3rd cup of coffee.⁣ ⁣
If you have dysfunction in your HPA axis, it is best to avoid caffeine while you are working to solve the problem. Caffeine can also increase the damage to a leaky gut, which is why most protocols eliminate it from the diet.⁣ ⁣

I did my self such a disservice eating a low-fat ,mostly raw, vegan, high carbohydrate diet. My banana green smoothies and endless date consumption was creating a highly inflammatory process that eventually led to  a disturbed #hpaaxis

While I am always searching for the root cause and trying to help patients resolve their issues in the long-term, there are situations where a quick fix is the first stepping stone, or can be the first stepping stone, towards that long process. ⠀

A great example of this is women who are post-menopausal and have extremely low hormones, and are having troubles with the motivation and desire to make the long-term health changes they need to in order to decrease all-cause mortality, and live a more healthy and fulfilling life. ⠀

Sometimes in these situations, looking at bioidentical hormones, for example, can be a really good option, short-term, to help these patients with the desire and motivation and willpower to start making healthier meals or get to the gym more often or have the confidence to get rid of unhealthy relationships in their life. In a situation like this, the hormones are a stepping stone to creating the long-term changes that could result in them being off their hormones, because their nutrition, lifestyle and psychology are all dialed into the place where they don't need those hormones anymore.⠀

Even though I preach and talk a lot about getting to the root cause, and really working on that to address those long-term issues and create long-term sustainability, sometimes we do need to look at short-term options to get patients feeling better quickly, so they can make those changes.⠀

When I was younger, I would always worry when my parents came home late. Did they get into an accident? Did they get robbed? Were the robbers then going to rob the house too? Would we be orphans? I would stay up until my parents came home which could be after mid-night and that was pretty late for a 12 year old. It was crazy what my mind conjured up

Maybe that’s why I chose to take up surfing so I can fight my worry demons. My last lesson was a tough one. I injured my left thumb just 15mins into my 2 hour lesson and I started imagining worst case scenarios in my head. What if I injured my right hand and I can’t use it as an Aesthetic doctor? What if the board hits my face and I break my nose? Seriously my worrying wild imagination needs a chill pill

But even with all that worry, I still pushed through. I don’t drink as much sea water now and I don’t have a panic attack as I used to every time I even get like 10 feet from the shore. I still worry about stuff and can be defensive when people offer me help because I’m in survival fighting mode so it’s work in progress

If it helps, here’s some tips to reduce worry:-
👊 Remember to breathe. 4 counts to inhale and 4 counts to exhale. This helps to better oxygenate the brain so you can think a little clearer
👊 Remember most of our worries are F.E.A.R - false evidence appearing real. Ask yourself “Will it really happen?” “If it does, is it truly the end of the world?” “If it did happen, what could you do about it?”
👊 Have a coach, teacher or mentor to support and guide you. When someone’s been through the path you want to take, they can help you minimise risk and maximise gain
👊 Tame your HPA axis - this is your fight or flight nervous system. Make sure it’s responding appropriately and not wired to look for danger everywhere. Hormonal balance can definitely help with this

HRV (heart rate variability) is an amazing tool to assess readiness to train and post concussion symptoms.

A healthy nervous system has a strong push/pull dynamic between sympathetic and parasympathetic systems leading to HIGH heart rate variability.
➖
Sympathetic dominance (fight or flight) leads to LOW variability. This can indicate you are under recovered or overtraining. Keys to recovery are sleep, nutrition, and stress management. Often low variability can indicate poor sleep or HPA axis dysfunction.

You can only compare your HRV to yourself as it is completely individualized. Having a good baseline is the starting point to compare throughout your training and daily life.
