To better understand my journey, and to help others in my situation, I’ll rewind a bit: time & place: the start of sophomore year in HS. In June of 2012, President Obama announced my beloved DACA, an immigration policy, that brighten up my existence in the US! It didn’t take long before I applied, couple months later, I held my future bright in my hands! With the permit, I was allowed to legally work, get my drivers license, attend college! I  would be able to finally enroll in those dual college classes my HS offered! I was the happiest girl alive! College was an option for me now; it truly was, prior DACA I wasn’t sure it was! Here’s the thing, I wasn’t informed and I was scared. I was the oldest, no one at home went to college, no one spoke of it. I would come home from school and tell my dear mami how amazing school was.  How I read, Of Mice and Men and learned of its themes, symbols and motifs, how I was able to jump higher during volleyball practice, how my teacher complimented my project but all I could see were my mami’s worries: the light bill, the rent, the water, gas, food, insurance. The worry grew in me. I knew once HS ended, it was my turn. The summer of my junior year I landed an internship (yess, at the clinic)! Senior year went by and I got called back, and got offered a position after an employee left! I took it without a doubt because of one thing I was sure: I HAD TO WORK TO HELP MY MAMI OUT & I HAD TO ATTEND COLLEGE,  NO MATTER WHAT!!

Tip #1: Know your why and value it! It makes the workload manageable! We all have our reason(s)! To all my first generation DACA students, I know your struggle, I’ve been there! It is hard: I went in blind. As an international student I had to take the AP Chemistry exam in May and get a 4 in order to meet my conditions and this is how I did it! It seems a bit messy but it absolutely worked. I was told multiple times leading up to the exam that putting information on my wall was a waste of time, but if an ‘ineffective study method’ helps you reach your goal then it isn’t ineffective!! •

Sauce: grass fed butter-higher omega 3 and healthier fats  and dry white wine (cut out extra residual sugars)

**cooking tip with olive oil-when cooking on high heat, you can actually turn olive oil into a carcinogen especially if using extra virgin so when we make dishes with heat we cook on low and will actually use just virgin Olive oil when cooking with any heat to prevent it turning to a carcinogen! .
Still add in fresh herbs like basil but the noodles make the dish a little lighter and you still get all the taste in the lighter white wine sauce ! #teamhnsfitness #cooking #cookingtip #healthyhacks

Conjunctival Melanoma. //Conjunctival melanoma accounts for 2% of eye cancers. It arises most commonly form primary acquired melanosis, or from a naevus (freckle) already present on the eye. Least commonly it arises on its own; this is known as primary conjunctival melanoma. This cancer can spread to other organs in the body, most commonly, the liver.
Risk factors:
The following increase your risk of developing conjunctival melanoma:
• Having pale skin
• Red or blond hair
• Blue eyes
• Over 50 years of age
• Large number of moles or freckles
• Poor immune system (e.g. HIV)
There are some features that increase the risk of the melanoma spreading. These include:
• Large tumours (large basal diameter, and tumour thickness of greater > 2mm)
• Nodular tumours (rounded and raised)
• Tumour involving the eyelids
• Tumour involving the corner of the eye near the nose (plica or caruncle)
If the tumour has grown on white conjunctiva instead of a pigmented conjunctiva, then the tumour may be more aggressive. Most common place is the liver. A liver ultrasound scan is arranged at your local hospital every year to exclude spread of disease.
Treatment:
Surgery (surgical excision) is performed with or without:
• Cryotherapy
• Mitomycin C (MMC)
• Radiotherapy
• Chemotherapy
Sometimes the tumour is too big or has spread behind the eye. In these situations enucleation may be the best option. If there is involvement of the eyelids or of tissue behind the eye, we may have to consider exenteration. If there is spread of disease to the lymph nodes in the neck, we may have to remove these or treat with radiotherapy.

If the procedure is successful, the patient could be a candidate for radiofrequency ablation of the pain fibers that supply the SI joint, for longer lasting pain relief!

I finished my cardiothoracic surgery rotation over a week ago, and last week I began my rotation in ophthalmology. 👓

In the past, the hospital that I worked at didn’t have affiliations with an ophthalmologist and students didn’t have the choice to do their rotation in a field like optho. I didn’t really want to be THAT person and change up a system that nobody had spoken up against for the past several years, but… I am THAT person. 💁🏻‍♀️ _

So I did it. Sent some emails, made some calls, maybe apologized a couple of times as I always think I am when I have to contact anyone in administration because I think I’m “bothering” them. 😅

And in the end it worked out! 👩🏻‍⚕️And guess what, it didn’t just work out for me, but now anyone doing their surgery rotation after me will have the choice to rotate in a field like optho— our director actually wants to make affiliations with other types of surgeons, too! I’m so happy with the outcome, and I think it goes to show that sometimes you may just have to do what nobody else has to make the changes that will help not only you, but others to come. This isn’t a shameless plug, but rather, a push for anyone who feels like they’re a “bother” when they try to make changes or do something that they feel like they deserve. 🙌🏽 .

Cool cases I’ve seen while shadowing this ophthalmologist? 👓 Herpes uveitis, choroidal melanoma, and steven johnson syndrome (with eye changes) would just be a  few of them. Even though this is my last week on surgery and the week of our exam (thus I feel more stressed than I feel present with patients), I am so grateful for the chance to be here. I find ophthalmology so fascinating. I wish this wasn’t my last week of eye surgery- but I am also so excited for my next rotation. Stay tuned ✨

1 cup of yogurt (I used original @ripplefoods)

The reticular formation is a mystery to many medical students (and pretty much everyone else for that matter) because it always seems to be represented as an arbitrary shape taking up the majority of the brainstem, as a way of representing its diffuse network of polysynaptic neurons. We are not sure if can be represented any better tbh! However, we can try and simplify it in terms of its main functions. Consider this an introduction to get you started- more reading is defo required to fully understand it! In general terms the lateral region receives afferent information and the medial region contains efferent projections. The functions can be summarised as the regions of the brainstem from where they originate. 1️⃣ From the medulla and pons we get the reticulospinal tract responsible for motor control, autonomics being carried to the cord, plus a little bit of pain modulation on top. There are also medullary centres for cardiovascular control thrown in for good measure 2️⃣ The reticular formation of the pons involves the medial longitudinal fasciculus (MLF) that is responsible for synchronised binocular eye movements, and some swallowing, plus some brainstem reflexes, which are super important when assessing an unconscious patient. 3️⃣ The midbrain and pontine area contains some raphe nuclei along the midline portion of the reticular formation. It has associated with it some ascending projections which make up the reticular activating system or RAS for short. This helps maintain attention and arousal during wakefulness by stimulating cortical areas and is frequently activated during painful experiences. The idea being that you have just been hurt by something, so you better wake up and move away from the danger quick smart before you get hurt again.
