Regrann from @drnadolsky -  If you looked at these plates separately you would probably think they are the same in terms calories. There is about a 150-200 calorie difference though, which can add up if you eat three meals a day.

I don’t think we should have to weigh our foods all of the time, but it may be useful if you ever get stuck or if you are beginning in your weight loss journey to get a better picture of serving sizes.

Multiply by three meals and you’d get anywhere from 400-600 calories of unaccounted calories.

Evidence based practice means that when faced with a clinical dilemma, a provider considers 1️⃣ Patient preference, 2️⃣the Best Available Research, and 3️⃣Clinical Experience to inform their decisions regarding rehab or treatment. By incorporating these 3 pillars of evidence-based practice into their decision making process, clinicians will avoid dogmatic practice behaviors, master their craft, and build rapport with patients to insure positive clinical results.

Being an evidence-based clinician is by no means an easier route, and it means that you will be consistently reading research🤓, spending more time with patients, and continuing to learn and improve upon the skills you already have. But it puts the patient’s needs first and that’s most important.

I came into this 6th week of my current training block feeling the effects of the past several weeks of hard training. I decided to start the week with a “Plan A” (continuing to push the numbers up) and a “Plan B” (call an audible into a low stress week if the performance wasn’t there).

Yesterday’s squat warmups felt slow, but improved on the way up - and with a bit of focus I was able to hit a pretty smooth 610 squat (though a bit higher than I’d like) before a bunch of back-off work and benching.
•
Today felt similarly flat at first, but I reminded myself that yesterday proved I’m recovering fine. Deadlift warmups felt faster and faster up to 585x4, and after last week’s nasty callus tear I decided to wear straps for this top PR set, before more back-off work.

Working my last overnight call shift (hopefully ever) this Friday night, then we’ll see how next week goes! Think it’ll be time to start pulling some singles soon 😈😈😈

That's 330 Lbs viz 158.76kgs lifted off the ground.
For a patient of low back pain ( L4-L5 disc bulge) associated with Sciatica, Deadlifts were a strict no-no. My orthopedic had absolutely banned any sort of forward bending exercises for me.
During the rehab for about 6 months I didn't go to the gym at all. Later when I started again with reluctance I ended up having that same pain again. This pain was like an irritating guest which kept coming and going. This continued for 3 years because of which I could not make a substantial gain in my physique.
My Mistake: I should have been more patient with the rehab.
A mistake is a boon if one learns from them and tries not to repeat it again.
So I worked really hard on my core and paraspinal muscles (only on body weight) 
My posture got better. I felt stronger and gradually I started lifting again, this time more controlled and balanced lifts.
Lesson learnt: injuries do happen, so never neglect core and spinal muscles because they're not visible. Your body is a temple so worship all of it in a proper method.

My goal was 20 total reps at 375 but ended up doing 4x6, followed by close grip, spoto press, incline close grip, total volume a little north of 35,000lbs plus some tricep accessories. #doctorsthatdo Talking to my patients, fellow colleagues, friends with full time jobs, parents, and relatives, they all have expressed their limited time for working out.  Be creative.  A gym and specific equipment are not required especially if you are new to working out.  Whether I am busy in clinic or doing a field training exercise, I use what’s around me.  For instance, as a military doctor, I am in the field a lot.  I will use a litter (army patient carrier) to do rows, pushups, crunches, etc.  15 minutes of a circuit is enough time to increase the heart rate to have an adequate workout.  Imagine doing this 5 times per week. .

Here is a workout plan (complete 4 to 6 sets): back rows until failure, pushups until failure, single leg step ups, crunches until failure.  And if you feel adventurous, consider carrying some weight. The following day, substitute the rows for pull-ups or pushups for tricep pushdowns, step ups for wall squats, crunches for sit-ups. . . .

It’s amazing when you actually care about patients how much better they do. When no one has actually listened to their story, their concerns and actually assessed them it’s no wonder opioids are over prescribed and many surgeries are performed unnecessarily. • • The world needs less cutting, less pills and injections and more listening and moving patients. This is so damn important for success and is also the reason I fight the gurus out there promising magical results.

Your weight loss is going to take time. Yes you can go faster in the beginning but don’t expect to be at your goal within a month. Shoot for at the highest 1% MAYBE 2% of your weight per week (in the beginning) and plan accordingly.

No detox drink is going to help you. Stop buying them. Stop buying those stupid wraps. Stop buying fat burners. They don’t work and/or they aren’t safe.

Snatches were starting to feel a little more consistent during this lighter EMOM session.  So now at least I'm consistently terrible at the same things, instead of randomly terrible at most things in the snatch.

I have seen a lot of people recommending knee scooters and crutches where you don’t use your arms to me . I don’t use them because I have knee reconstructive surgery on the other side and am afraid of getting a blood clot, or DVT. Part of the reason I work and workout is we know your risk of blood clots from a DVT to pulmonary embolism increases with injuries, surgeries, fractures and if you have underlying medical conditions. So much so- my orthopedic surgeon warned me about risk while wearing the boot. To prevent it I try to keep the rest of my body working 💪

It is a delicate balance between taking it easy and working too hard after an injury. I am thankful to say I’ve tired myself out after my surgeries and injuries but never gotten a DVT due to likely genetics and staying active. DVTs are incredibly common and even more so with immobilizers and there aren’t enough studies to know who is at risk with these new devices and if blood clots are more common with them . Food for thought.

Rotator cuff has been bugging Me for over a week and it came up in conversation with @maggie_l_kuhn and without missing a beat hooked me up with this and less than 10 minutes later my pain and tightness was gone!! Talk about a great gym from the equipment all the way down to the people.  You can't get any better than this!

So that’s what I did ( and got to train Mama Carma in between sets 👸🏻🤗🙌🏻) here’s a simple sumo deadlift a few lbs above my body weight. Couldn’t move 90lbs without pain a few months ago and now 125lbs for 4 sets of 6 is smooth as butter. Happy Sunday 💕

I regard strength training as moving meditation, something very similar to martial arts. It is a way to test the physical body that was given to me. It requires full concentration, especially as the movement is more difficult, or the load is heavier. To my female friends, colleagues, and patients—don’t be afraid to lift, and don’t be afraid to lift heavy. Exercise is the most potent yet underutilized form of medicine. Luckily at @starkirvine, that is usually not the problem. Oh, a few patients asked what I usually eat after a workout. BEHOLD—today was 8oz of wild scallops. 🤤

Amazing day at Georgia's Strongest Man, an extremely close competition that came down to 0.4 lbs of bodyweight. But I got the win and am officially Georgia's Strongest Man 2018 MWM. Bittersweet moment because it is probably the last time I will be able to compete in the Georgia and be this strong for the next 4 years as residency gets under way. But I am very happy nonetheless. Hit a 10 pound PR on axle clean and Jerk at 325. Hit a huge PR by axle deadlifting 605 for 4 reps.

Thanks to Scott Helms for putting  on another great contest and to all the loaders for helping all day long. Thanks to my mom, @tlcampbelljr , and @catherinegarrett for coming to cheer me on. For some reason, the word “organic” has become synonymous with “healthy.” Unfortunately that doesn’t really pan out (example above).

In fact, a common misinformed argument is that “organic foods don’t contain pesticides.” But the USDA classification of organic is based on using approved ✅ substances (natural vs synthetic), not complete lack of pesticide exposure.

Organic products don’t carry much (if any) inherent health or weight loss benefits over conventional products.

In the example above, the peanut 🥜 butter cup calories and macronutrient make up are almost exactly the same. Both are calorie dense and nutrient poor.

Organic products do, however, tend to cost significantly more than conventional counterparts.

If you’re trying to eat healthy, especially on a budget, choosing organic over conventional products may not be worth it 🤷🏻‍♂️

You don’t necessarily have to avoid yummy treats like these if you can fit them in to your overall daily caloric allotment. But be aware that $3 organic peanut butter cups are not healthier than $1 regular ones.

I think it’s very important to listen to your body. Today, I woke up feeling kind of off. No pain but just not right. I had three choices: 1. Skip the gym and rest. 2. Go to the gym, still workout and decrease my load. 3. Say screw it, go hard and heavy and risk the injury.
I chose number 2. Which would you do?

The only time I ever make my bed is when I'm on call.. Gotta prep to make myself feel better. 😂

It was a moderately busy week on inpatient this week. It's good because it took me a few days to get back into the swing of things and get over my jetlag from Dubai/Abu Dhabi.

But I gotta tell you... As you've probably noticed, I've been slacking hardcore with my workouts. I haven't been consistent with it mainly because I didn't commit to a program. And with traveling and just the intensity of inpatient rehab unit {yes, real rehab is actually busy, contrary to popular belief that PM&R stands for "Plenty of Money and Relaxation"... Depends where you're at}, it's been rough. ---
SO ... With that being said, I'm determined to get back into it starting Monday. I've been super excited to announce a brand new program that's coming out that was designed specifically for busy people who "don't have time" and/or "don't have space" to workout... It's called LIIFT4 which is an 8 week weightlifting and HIIT combo program to build muscle and lose fat. It's only 4 days/wk for 30 min/day {you get weekends off!} with cheat days built into the nutrition plan. 
I'm hosting a VIP launch group for this brand new program, which means you will have exclusive access to the program before this is released to the public on October 1st! I like to use lift variations with pauses in different positions in my training and when rehabbing athletes for lighter training days and to help improve positional strength. For instance, if an athlete has difficulty maintaining, or discomfort with, the overhead position in a snatch I will include lighter load lifts with 2-3 second pauses in the receiving position. The goal during these lifts is to achieve the best possible positioning and midline stability.

I think we can all agree the human body is able to adapt, pain is complex, and we aren’t fragile. But to say we aren’t fragile and posture and technique doesn’t matter is not completely correct either. •

I love pain science, I’ve read a lot of pain science, I don’t claim to be an expert on pain science, but I do take issue when people are so quick to just say something is wrong because it’s not worded in the new sexy pain language context. Which in general pain science is framing, this framing is also going to need to be individualized to each patient. IE engineers typically don’t understand vague descriptions of pain without biomedical language because that’s how they think vs some people can understand their spine issue without talking about a disc etc. •

I adapt my language to my audience just as any good therapist should, as we are educators first, therapists second (and with most issues we should be load masters). However, there is overwhelming data that the majority of what 1st world countries do is not awesome for their spines, and rooted in the bio aspects of a BPS model does have to do with repetitive motions and poor body awareness. Most people spend all day bending, reaching forward, lifting and sitting slouched. How’s that working out for people? Well spine related pain is the WHO’s 5/6th most burdensome health issue (not ranked higher only because people don’t die directly because of it). •

Clearly we are not doing something right. And just going forward spine pain doesn’t just mean your back or neck hurts, since the unique thing about spinal issues is that it can create symptoms anywhere and everywhere the nerve root at impacted segments travels to. The thing is people need to learn how to load their spine appropriately, and yes it should be framed in a non threatening way. But we can sit here and say posture and form don’t matter until it does matter!•

Like I get it there is no one posture that is perfect for everyone, as well as lifting technique. We all have different limb lengths, curves in our spines and just overall differences anatomically. •
